Improved Clinical
Outcomes
LifeLink Monitoring services improve clinical
outcomes in disease management, at the same
time that they increase operating efficiency.
Conditions such as congestive
heart failure and diabetes
require the rich and timely combination of
objective and subjective data that our telemonitoring
services provide. Immediate Interactive Voice
Response (IVR) feedback and symptoms
questions reinforce the importance of
lifestyle management and increase patients’
knowledge of their condition and awareness
of early signs of trouble.
High risk patients can
monitor and send readings daily. Individualized
alerts help case managers decide which patients
to call, enabling them to manage by exception.
Instead of wasting time calling all the
patients in their panel to collect vital
signs and symptoms, clinicians can focus
on patients who need help, when they need
it. Telemonitoring has been shown to dramatically
reduce hospital and emergency room utilization
in congestive heart failure and childhood
asthma.
Our
blood pressure telemonitoring service
is the only cost-effective telemonitoring
service for hypertension
disease management. By avoiding unnecessary
treatment costs for patients with white
coat hypertension or white coat resistance,
home blood pressure telemonitoring shifts
resources from patients who do not need
them to those who do. Telemonitoring in
true hypertension improves medication compliance
and rates of blood pressure control.
There are telemonitoring
equipment manufacturers serving the disease
management industry, but we are the only
company that offers a scalable pay-as-you-go
service, with no upfront investment
in costly equipment and software, and no
long-term leases.
We handle all the details:
delivering monitoring kits to patients,
training them to install and use the system,
managing the flow of data around the clock,
reminding patients when they forget to monitor,
distributing raw data and reports to case
managers and physicians, helping patients
if they have trouble with non-clinical issues
in monitoring, getting the kit back at the
end of the episode, and cleaning, calibrating,
repairing and replacing the equipment. We
take care of the hassles, freeing your staff
to devote their time to what they do best
– disease management.
Financial Considerations
We do not sell or lease the telemonitoring
equipment to you. We own the equipment and
provide it to your patients as part of our
service. That means you have no front-end
investment in monitors, software or servers.
It also means that you pay only for the
telemonitoring services that each patient
needs, and only for the months covered by
the episode of care.
By not requiring you to
buy or lease any hardware, we shield you
from several business risks, including the
front-end impact on your cash flow, equipment
maintenance, calibration, and refurbishing,
technology obsolescence, and any misalignment
between forecasted and actual demand. If
you were to own or lease a fixed number
of telemonitoring systems, you would run
the risk of not having enough units to meet
your needs, or of having unused equipment
sitting on your shelf and incurring lease
charges.
With LifeLink Monitoring,
you pay only two fees: a one-time enrollment
fee for each patient, and a monthly service
fee for each patient as long as they use
the service.
