TELEMONITORING

Our vision is to make telemonitoring a standard of care, available to all clinicians and patients who want to reduce the terrible health and economic burden of poorly-managed chronic diseases. With this goal in mind, all of our telemonitoring services are reliable, low in cost and easy for patients and clinicians to use. We do not require large investments in costly equipment, and we do not force people using our service to adopt new and unfamiliar technologies.

How It Works
Our telemonitoring services enable patients to monitor blood pressure & heart rate, weight, blood glucose and hemoglobin A1c at home, and to send objective data to clinicians over the telephone. Patients also report their subjective symptoms and medication compliance.

After patients send data, they pick up the telephone and an Interactive Voice Response (IVR) system tells them their recent readings, and congratulates them if they are at goal.  The IVR also asks questions about symptoms and medication compliance.

Within minutes results appear on our patient reporting website or directly interface with electronic medical record or case management systems.  LifeLink Monitoring services are easily integrated into a wide range of clinical and IT environments.  

A primary care physician may elect to check the website weekly to make treatment decisions for non-acute patients with hypertension or diabetes.  For a pregnant woman with hypertension, blood pressure readings can be checked daily.  A case manager will have home monitoring data from congestive heart failure patients on his or her computer within minutes of the patient sending readings.  This daily home surveillance frees the case manager to focus on the sickest patients, secure in the knowledge that daily telemonitoring provides rapid notification if another patient is getting into trouble.
